Our therapy success story for January is Mr. Charles Frazier. Charles came to the facility for short term rehabilitation after a hospitalization with a significant decline in self care, mobility, and overall functional independence at home. Upon admission, Charles was dependent in all skilled areas and overall deconditioned with task. Charles gave 100% daily during his therapy sessions and progressed quickly with daily motivation from his wife. Pt was discharged min/mod A with all self care and mobility and returned home to live with his wife. Thank you Mr. Frazier for allowing us to serve you.
We are happy that you are going home but we sure are going to miss seeing your smiling face in the facility!!
